Please indicate which times you would prefer to train, which times you are available but may not prefer, and which times you are unable to train on a regular basis.  Responses on this form do not commit you to a particular day/time.  You may choose to meet your personal trainer as infrequently as once a month or as frequently as multiple times a week depending on your goals, schedule, and fiscal flexibility. Actual training day, time, and frequency will be decided between you and your trainer and can be revised at any time during participation.
